We welcome your request for a price estimate related to your upcoming treatment at EvergreenHealth!  Call to speak to a member of our Patient Financial Services team, who will provide an estimate* based on your services and your health insurance provider, including the specifics of your coverage plan. 
Things to know about your request…
  • It is helpful to know the CPT code of your procedure (your provider can give this to you). If your procedure is already scheduled, our staff may be able to help get this code for you.
  • Please allow up to 24 hours for some estimates as it may take extra time to obtain certain information related to your request.

Have Insurance? Start Here.

If you have commercial health insurance, your insurance company can help you obtain an estimate of what you will pay for a specific procedure. Your insurance company, even more so than our resources, is often the best place to start. Your insurance provider can determine out-of-pocket costs in advance of obtaining services, since it knows the specifics of your coverage, plan, deductibles, and co-pays. Some insurers have online tools and estimators, while others will ask you to call and speak to someone.

No Insurance? We Can Help.

If you do not have coverage, we can assist you in obtaining medical benefits through federal, state and hospital programs. These programs may help you with your current bill and will help you pay for health needs in the future. We will help you find out if you qualify for charity care. In some instances, we may be able to provide free or reduced price care if you qualify based on your income.
